HYALURONIC ACID I The right strategy

Zone by zone or global treatment with hyaluronic acid?

We perform filler injections with hyaluronic acid gel zone by zone.

Thus, the following can be injected: cheeks, cheekbones, dark circles, lips, nose, chin, jaw, etc.

But in a more intelligent way, we can target a anatomical region : facial injection, hand injection, plumping up the intimate area, smoothing the neckline.

Or, reach a aesthetic purpose thanks to fillers: A rejuvenated face thanks to hyaluronic gel, rejuvenation of the hands by filler, a more virile face, a more glamorous smile, a less visible scar or defect by skin booster.

I need to see you in consultation to understand your project and build it together.

Hyaluronic acid is naturally present in the skin

Hyaluronic acid is present in the body in natural form.

In its natural form, hyaluronic acid is non-crosslinked. In this form, it hydrates the skin by micro-injections.

The fact of being cross-linked in the laboratory improves its duration over time. Depending on the desired goal and the indications, the doctor will choose the product most suited to the level viscosity.

Thus, you need a fairly fluid gel for the fine crow's feet wrinkles. Furthermore, it is recommended a more cohesive gel for wrinkles in the folds of bitterness or the nasolabial fold. On the other hand, the cheeks will benefit from a product with a strong volume effect to create matter.

Filler prices

We do not charge per syringe…

This “syringe” concept is outdated because several densities of product must be used for the same area. Thus, we give a price depending on the objective to be achieved, according to the wishes of the patient and the diagnosis made in consultation. A real embellishment by injections requires a financial investment annually otherwise the result will be insufficient.

Rejuvenation or beautification: I answer all your questions without taboo

Is hyaluronic acid a medical device or a medicine?

Hyaluronic acid is a medical device. The legislation is therefore more flexible than medicines, we can advertise subject to professional information. On the other hand, cross-linked hyaluronic acid used for volumizing injections can only be used by a trained and competent doctor.

I would like a natural result, what should I do?

Whether or not the injection is natural depends on the philosophy of the cosmetic surgeon contacted for your project.

Indeed, Dr. Smarrito has advocated and taught respect for anatomy for many years. The injections allow for beautification and rejuvenation but certainly not the current trend towards transformation. Everyone has seen overly swollen lips, impressive cheekbones, distorted and lunar faces on social networks or on television shows.

It’s not medicine, it’s transformation. We must avoid this type of practice and not give free rein to those who practice these excesses at the expense of patients. An injection must be respectful of the anatomy, the volume must be controlled to avoid complications. In case of excess, the complications are: compressions. In addition, the aesthetic result does not depend on the quantity but on the shape.

Yes ! It is perfectly possible to have a natural result that will delight you in front of the mirror. This result will please your family and will add an aesthetic touch. You can also then move towards a sexier appearance but certainly not one that transforms you to the point that people don't recognize you.

Price of hyaluronic acid injections

Very often, patients call Dr. Smarrito's secretaries to find out the price of a hyaluronic acid syringe. We understand this approach perfectly, but it is no longer in keeping with the times. Indeed, many clinics advertise, but the person who performs the injections is completely hidden behind the sign, or even non-existent. Thus, the selling point of these clinics is mainly a low price, or even frantic competition between Lausanne clinics to see who will get the lowest price of hyaluronic acid. This is not the right tool to compare doctors, or clinics, because there are many laboratories, many products, many different qualities of hyaluronic acid and thus many prices. You can have an injection at low cost. price, with a low-end product, whose effectiveness will not be established over time, or even risks side effects such as granulomas.

It is often better to prefer to have a fair price.

The value of the injection does not only come down to the product. Doctors are not pharmacists who deliver a product, doctors first and foremost make a diagnosis and, depending on the indication, will choose the appropriate product. For example, in the Stylage range, certain products are suitable for superficial injections and others for deep injections. The price scale for purchasing products ranges from single to double depending on the indication and specificity of the product.

Dr Smarrito will receive you in a classic aesthetic medicine consultation and will then offer you a global quote, including one to several injections, or even several sessions over time. Therefore, the secretariat will not answer the question of the price per syringe. A package is put in place. We strive to ensure that the price is as correct and as medical as possible.

It should be noted that in terms of price, if you want to obtain a result with hyaluronic acid, you have to pay the price. It is a real financial investment because the surgeon must be able to purchase the most effective product and above all inject the quantity of product, or even the number of products necessary in relation to your objective. Otherwise, the objective will not be achieved and the result will be disappointing, or even unstable over time.

For any information on the price, please contact the secretariat. A consultation will be carried out and at the end of the consultation, a precise quote will be given to you.

Russian lips or Bi-Bi lips?

Dr. Smarrito hates Russian lips.

Russian lips is a term designating the over-correction of the lips for a so-called glamorous effect, very trendy at the moment, especially on social networks and for the millennial generation.

This technique is not respectful of anatomy. In fact, it will distort the different lines of the anatomy. In this context, Dr. Smarrito conducted a study, over more than 3 years, starting from the dissection of cadavers after injection and reflecting on a new philosophy of lip injection.

Russian lips, and also called "BEE-STUNG Lips" and going to the extreme "Heart-Shaped Lips", even delusions like the devil's lips where doctors, if you can call it doctors, inject the lips to make 2 points like the devil's horns.

The Bibi Lips technique is a serious technique which respects the 2 anatomical planes, which are the dermis and the muscle, which uses a needle on the surface and a cannula in depth so as not to injure the artery of the lip, which plumes the product with a light product (Stylage Lips) on the surface to give a beautiful contour line to the lips and a product underneath (Stylage Lips plus), which is more volumizing.

A study published in an International journal, led by Doctor Smarrito, showed the absence of complications after these injections and good stability over a period of more than a year. Furthermore, the term Bi-Bi also refers to the ceremonial hat which is a signature of French elegance. This play on words is significant since in fact, the aim of this injection is to give charm and character to the lips but not to have inflatable doll lips.

Dr. Smarrito does not practice the Russian Lips technique, as you will have understood.

What is the difference between hyaluronic acid and botulinum toxin?

Hyaluronic acid is a medical device, it is a gel like modeling clay which will hydrate the dermis and sub-dermis, but also give volume to the card, i.e. a micro-volume for fine lines or wrinkles, or important modules to reshape a cheek, raise a zygomatic arch, sculpt the contour of the mandible (known as Jauline), improve the aesthetic appearance of the nose, beautify a lip or quite simply plump up the hands or rehydrate the décolleté.

Botulinum toxin, whose best known brand name is botox but which currently has other trade names such as Belotero or alluzience or Azzallure is a medicine, in this sense it is governed by the rules on Swiss medicines and SWISSMEDIC therefore, advertising on this product is prohibited.

Botulinum toxin has a completely different mechanism of action from hyaluronic acid. It is now a synthesized product that can modulate muscle activity. It is of course possible to block the muscle, leading to a reduction in contraction and therefore mechanical wrinkles, in relation to the muscle, but also to simply modulate muscle tone to soften the appearance of micro-fine lines, or facial wrinkles. light expressions.

Only doctors can inject these products. Dr. Smarrito has great expertise in the practice of both botulinum toxin and hyaluronic acid. Note that sometimes you have to combine the 2 to obtain a result.

The use of botulinum toxin in wrinkles requires the advice of a surgeon because sometimes muscle contracture is just a defense mechanism in relation to excess skin or drooping of the same, as in the case of the eyelids.

Very often, forehead wrinkles are due to the need for eyelid surgery. Injecting botulinum toxin into this area can lead to very unsightly drooping of the eyelids. It is advisable to practice botulinum toxin with a cosmetic surgeon, as well as hyaluronic acid, to have an overall opinion and know all the possibilities in relation to your facial aesthetic project.

What precautions should be taken before a filler injection?

A consultation is carried out and the doctor must be informed of any medical element that could constitute a contraindication, such as pregnancy, the presence of an autoimmune disease, taking products that cause bleeding or anticoagulants, a chronic illness or a recent illness.

It is also very important to establish a list of injections that have already been performed. Some produced in the past can be reactivated in the form of granuloma during a simple injection of hyaluronic acid, for example aqualyx which has a tendency to produce granulomas anticipated and which should not be reactivated during injection . The exhaustive list of products injected into the areas during the project is very important.

Taking arnica is possible, but arnica, used in homeopathy, thins the blood. The use of helicrysis will be much more interesting post-injection to reduce the effects of edema or even blueness.

It is advisable to purchase a mask with beads to apply cold after the injection and some compresses which are moistened to provide relief, particularly during lip injections.

You absolutely must not be pregnant or breastfeeding during the injections. This is a simple, understandable security measure.

Dr. Smarrito's team will be able to inform you perfectly if you have any doubts about the precautions or contraindications. On the other hand, the medical decision rests exclusively with the doctor in consultation.

Dr. Smarrito’s tips after injections

Depending on the area, small procedures will be implemented to reduce edema, promote comfort and obtain a more stable result over time.

As always, reducing or stopping smoking is favorable because the more the tissue is well vascularized, the greater the water supply to hyaluronic acid and the more generous and stable the effect of the product will be over time.

We use homeopathy a lot, rather post-injection, to reduce transient edema, or even erase possible bruises or bruises, thanks in particular to type preparations. helichrysum (immortal flowers, the Curry plant).

Dr. Smarrito’s team remains available for any advice or any surprise following an injection. Furthermore, there are safety measures to be respected and instructions will be given, particularly in the event of unexplained pain, whitening of the area or worse, the appearance of a dark spot which may suggest a vascular complication. .

Are the results of the injections instantaneous?

No. First, slight swelling is possible just after the procedure with hyaluronic acid, particularly in the area of the lips, which can swell profusely for 24 to 48 hours. You have to anticipate it from a social point of view and also warn those around you, this is not the definitive result. The result will be much more stable after a few weeks. If something seems abnormal to you, a touch-up is possible, it is often carried out every two weeks to adjust.

Note that injection cannot correct problems of lack of symmetry. Anatomy is always asymmetrical and a product such as hyaluronic acid does not have enough strength to correct tissue or morphological asymmetries often due to bone relief. It is therefore necessary to plan for over-correction for the first 2 days, which will fade, and sometimes a bruise on the injection area which will take between 1 to 7 days to fade. Luckily, it is possible to apply makeup.

When can I re-make up the area?

It is entirely possible to reapply the injection area immediately after the injection. You just have to be careful not to put foundation in the area where the cannula or microneedle is inserted so that the product does not enter the area. Note that this area will close in a few hours and it is then perfectly possible to apply makeup or camouflage in the event of edema or bruising.

Many brands have a professional range of post-procedure makeup. Note that the green correcting sticks are very interesting because they remove the redness and then allow the application of a coverage type foundation by Avène, for example, corresponding to your skin which you can choose at the pharmacy in order to to camouflage the area during the transitional phase where visibility would be greater.

Are there any charges for touch-ups during hyaluronic gel treatment?

Dr SMARRITO checks patients after the injections if necessary or systematically depending on the project which is established during the first consultation. Touch-ups are free. On the other hand, if it involves adding product, increasing the volume or obtaining a result different from that desired during the previous injection or having a global project over a year to magnify the appearance for example of the lips or the entire face, it is no longer a question of touch-ups but of new procedures or supplements which will be billed or which have been integrated into an annual follow-up formula. Do not hesitate to discuss with Dr SMARRITO if you would like a one-off injection or if you are investing in a gradual improvement in the beauty of your face, hands or décolleté with several sessions considered.

Are there allergies to hyaluronic acid?

Allergies to hyaluronic acid are extremely rare. It is mainly a swelling, an edema, a sort of local hyper reaction. They are often due not to the hyaluronic acid itself but to the excipient or the liquid which contains the hyaluronic acid.

These reactions are very rare, but they do exist. If you have a known or even documented situation, you must indicate this during the first consultation, the doctor will then be able to assess whether a test is necessary before carrying out the injection. In most cases, these extremely rare reactions are mild and do not require hospitalization or specific treatment.

In more serious cases, the office is equipped to counteract any excessive allergic reactions. Allergies are distinguished from side effects such as compressions or intravascular passages of hyaluronic acid; they must be immediately corrected by the injection of the antidote which is hyaluronidase. Likewise, foreign body reactions (inflammatory shell formation) lasting over time are extremely rare with hyaluronic acid in general.

Dr SMARRITO has been using STYLAGE hyaluronic acid for many years and is also a world reference on the subject, participated in the development of product safety instructions and has never observed a granuloma on STYLAGE.

Why talk about acid?

Dr. SMARRITO admits at world conferences that when he was a young surgeon, he was impressed by the term "acid" and didn't quite understand why acid was injected under the skin to create volume. In fact it is the purely molecular description in organic chemistry of the spatial structure of hyaluronic acid.

In fact, from the year 1934, Karl MEYER and John PALMER, from the Department of Ophthalmology at Columbia University, isolates a new sugar in the vitreous humor of a bovine polysaccharide and combines two chemical terms from its structure to name it: uronic acid and hexosamine. So, they call this polysaccharide “hyaluronic acid” (this comes from the Greek hyalos = vitreous and uronic body because it contains a lot of uronic acid). In fact in the body this acid behaves like salt (sodium hyaluronate).

Hyaluronic acid is therefore a “sugar” which is worked to be injected safely and also to have good stability in the body.

On the other hand, patients will not feel acidity during the injection. It is the opposite, for example of anesthesia products at the dentist which, due to their acidity, generally creates pain during injection.

There is therefore no need to fear this word, which is just a description of the arrangement of the molecules forming hyaluronic acid in space.

How long does an injection last?

Consultation is important. Dr SMARRITO will always carry out a 1st consultation before performing the injections. It makes it possible to determine the project, create a medical file and implement a global strategy. In fact, hyaluronic acid injection is reserved for doctors in Switzerland since the product is absorbed in more than 30 days. It's the law. But experience shows that the diagnosis is much more important than a simple choice of the type of syringes or brands of syringes since the level of expertise of Dr SMARRITO will make it possible to mix the products to obtain the result. Furthermore, sometimes Dr SMARRITO will advise against the procedure, either from an aesthetic point of view or from a purely medical point of view. Apart from this consultation, the injection lasts between 10 minutes and 1 hour. It is easy to understand that 10 minutes generally corresponds to the injection of a single syringe and 1 hour can be a more complete work on the face, on the hands, on the neckline with a proposal for multi crosslinked or non-crosslinked injections.

Who makes hyaluronic acid?

Hyaluronic acid is not made by the doctor. It is manufactured in a clean room factory by certified laboratories which are generally rather specialized. You should be very careful not to buy hyaluronic acid on the internet because fake products are circulating. In the boxes of STYLAGE type hyaluronic acid, a label is present which is used by the surgeon to monitor and have the batch of the product, which is normal in terms of industrial traceability, but which also contains magnetic lines which attest to the character original of the product. If non-crosslinked hyaluronic acid can be mixed with other products such as vitamin E, DMAE, MCTF, it cannot be properly synthesized by the cosmetic surgeon.

What is the difference between the hyaluronic acid contained in creams and that in injection syringes?

In creams, the hyaluronic acid present is non-crosslinked. It is there to provide hydration which in the case of the cream remains very superficial, since it will not go further than the simple skin barrier and the dermis.

When it comes to injections, hyaluronic acid is cross-linked or non-cross-linked. The non-crosslinked ones are used in mesotherapy treatment mainly for their hydrophilic powers which allow good hydration while the crosslinked acid will rather be used for its volumizing effect. It is not possible to inject a cream based on hyaluronic acid. Furthermore, the concentrations of hyaluronic acid and the molecular weight of hyaluronic acid are very different between creams and those injected. Very few hyaluronic acid products are actually used in creams while in injection most of the time it is pure hyaluronic acid which is used for an effect a thousand times greater than an application of cream in surface.

Is there legalization in Switzerland of hyaluronic acid?

Yes, there is legislation that governs the use of hyaluronic acid since it is a medical device and is strictly reserved for the doctor.

Dr. Smarrito conducts numerous training courses for future doctors injecting hyaluronic acid around the world. He can attest that training is essential not only for patient safety but also to know the different products to use and last but not least to have a professional result corresponding to the speech in consultation.

Any medical procedure requires prior medical consultation and also the ruling out of an illness that would be incompatible with the use of hyaluronic acid.

In Switzerland are beauticians authorized to inject hyaluronic acid?

According to publications from Swissmedic and the health authorities of the canton of Vaud as well as the cantonal doctor, beauticians are not authorized to inject hyaluronic acid.

In fact, on the Swiss market, no hyaluronic acid is absorbed in less than 3 days. A beautician can inject a product which will be absorbable, mainly water-based products but not hyaluronic acid-based products.

The use of a hyalupen, that is to say a high pressure injection device without needles, is no exception to the rule since it is an injection.

A lot of information is available on the website of the Swiss Society of Aesthetic Medicine as well as on the website of the canton of Vaud. Unfortunately, many proposals are visible, particularly on social networks.

Hyaluronic acid injection can be carried out by a professional to have a satisfactory and stable effect over time. Furthermore, the risks of an injection are low provided you know the anatomy. The great danger of performing an injection by a beautician is that she risks injecting into an artery which can lead to occlusions or necrosis.

Furthermore, if we reduce the price in relation to the duration of the products and the effectiveness, in the end, a gesture carried out by a doctor or by Dr. Smarrito who is an expert will be much more profitable since there will be a real efficiency and above all less need for rework and reinjection over time.

Hyaluronic acid injection can be carried out in a basic way but there can also be very sophisticated refinements and techniques to achieve the desired effect.

Is hyaluronic acid resorbable or is it part of permanent products?

Hyaluronic acid is an absorbable product. It is present spontaneously in the body in which it is resorbed thanks to the action of the hyalase but also following the degradation due to the presence of free radicals. In a way, hyaluronic acid has a natural side in the body since it is used to moisturize the skin and give volume to the skin and sub-skin.

Non-absorbable products have generally been abandoned on the face because the body often interprets them as a foreign element and tries to defend by encapsulating them, creating inflammation, trying to reject them by macrophages. This foreign body reaction, unfortunately very well known on products from the past which were launched on the market with great publicity to last longer than hyaluronic acid, sometimes leads to catastrophic situations, since granulomas are very complicated to to treat.

The resorbable side of hyaluronic acid is a clear advantage for Dr Smarrito since it allows the treatment to be adjusted throughout the sessions, to the patient's taste. Furthermore, if the volumizing side of hyaluronic acid does not please the patient, returning to square one is possible and there will be no skin deformation thanks to hyaluronic acid, at least during the first sessions. Thus the resorbable side of hyaluronic acid can also be experienced as a possibility of adaptation, which is fortunate. On the other hand, the use of hyaluronic acid sometimes requires several injections (several syringes per session) to obtain the expected result, or even several sessions over time.

The only difficulty with the resorbable side of hyaluronic acid is in fact the cost, since you have to pay again for a new injection. On the other hand, by combining the sessions the price is often much lower than that of surgery.

Dr Smarrito is an FMH aesthetic doctor who will be able to respond to your request with surgery but it is another philosophy of the use of filler.

Does Dr. Smarrito use cannulas or syringes for injections?

This question is often the subject of fierce debate at conferences. Dr Smarrito is lucky enough to often be the moderator at conferences like IMCAS, which is a false problem!

Each tool, syringe (flexible or rigid) or cannula (micro-cannula), needles (short needles, long needle), has advantages and disadvantages, but strictly speaking it will also depend on the indication.

Needle work is often more precise and allows droplets to be delivered to a very precise area. On the other hand, it is obvious that a needle is more traumatic than a cannula.

The cannula provides an element of safety, if we are in contact with vascular areas since above a diameter of 25 gouges publications show that there is little risk of having a vascular break-in.

We are very attentive to this element since vascular invasion can lead to tissue loss, necrosis, embolization inside an artery which are one of the injection risks which an expert can safely avoid. emergency but which requires a certain know-how.

In order to avoid these incidents, the use of a cannula, as soon as you are at depth, seems safer to the entire medical community.

The choice of tool will in fact depend on the desired purpose of the area and the comfort of the patient. For example, in the BIBI Lips technique, invented by Dr. Smarrito, Dr. Smarrito explained why on the surface it was necessary to use a needle while at depth, a cannula is more suitable.

Thus, one must not have a stereotypical attitude when it comes to hyaluronic acid injection but, in a way, have the know-how and intelligence to adapt the product and the tool to the needs of the patient. and especially to the anatomical area.

Can we inject a patient on antibiotics?

It is very important before carrying out an injection, to have a medical consultation in the event of medication or new medications compared to previous injections, this must be indicated to Dr. Smarrito.

Most of the time, antibiotics do not contraindicate hyaluronic acid injection, however the disease for which the antibiotic is used may be a contraindication.

Certain medications, on the other hand, are contraindicated, such as medications that can cause bleeding, typically aspirin, anti-inflammatories, yes those very clearly prescribed to thin the blood and prevent clotting, such as anticoagulants.

It is sometimes possible to deviate from this rule, but the injections must be superficial, rather mesotherapy type injections, because the risk of deeply injecting a patient who has anticoagulants could be a risk of very significant hematomas at the level in particular. of the muscular area which at best will give an unsightly appearance which will last several weeks and at worst can lead to a real formed hematoma.

You should therefore always inform your surgeon if you have changed your treatment or if a new pathology is being treated.

I have cold sores, can I have lip injections?

Yes, it is entirely possible to have an injection around the lips in case of herpes, however a prior consultation, a few weeks before the injection, is favorable since in this context the doctor who is aware of your history will be able to prescribe medication and an ointment, since herpes can appear the day before the injection due to stress or just after the injection, given that the needle or cannula can still have a traumatic effect .

Likewise, if a herpetic outbreak appears on the day of an injection, in the area or very close to the area, it is better to postpone the session. You can come to the consultation and the doctor will then prescribe the appropriate treatment if you do not have one as a routine.

I had silicone injections, can I inject hyaluronic acid in the same area?

Dr. Smarrito, as an expert, strongly advises against injecting hyaluronic acid, or any product, into an area that has been injected with silicone or that currently presents a foreign body granuloma effect.

Indeed, any injection can reactivate inflammation and therefore accentuate the side effect of these non-absorbable products, which have led to encapsulation or a deleterious effect.

Please note that these products are currently banned in Switzerland. It is absolutely necessary to ban any injection of silicone into the tissues. Only silicone in breast prostheses is interesting, but the technology is totally different.

In the past, liquid silicone injections were performed, particularly on the lips. The body perfectly identifies it as an exogenous element and fights it fiercely, resulting in swollen, red and inflamed lips.

Often, this is an extremely complicated process for patients, since there is strictly speaking no solution to get rid of the silicone injected into the dermis.
