Who invented hair transplant?

It was in the 50s that Dr. Nobuto Takahashi performed hair follicle transplants to treat baldness. Other techniques had been tried before, but it was from his work that hair transplantation took off.

Grafting has been greatly improved, moving from large grafts (with the appearance of a hair doll) to currently being performed in the form of single micro-grafts using the FUT or FUE technique.

I also experienced scalp band relocation surgery when I was a young intern…not great!

Modern techniques are reliable and effective, but you need to consult an FMH surgeon to get the right advice and a coherent strategy.

What is the difference between FUE and MCE?

FUE grafting is a Follicular Unit Extraction graft, the concept of which is to make small holes in the back of the skull in the donor area and then transfer the grafts to the recipient.

These are often small holes of about 07mm or 08mm, containing a hair and a graft. This technique is very effective, we perform it using the FUE SAPHIR technique, it allows to manage localized areas or to make larger areas without leaving scars.

The MCE technique developed by Dr. SMARRITO is a very interesting technique because it allows a much larger supply of grafts during surgery, without leaving traces on the scalp, provided that the procedure is carried out by Dr. SMARRITO.

This method will on average give more than 5 hairs and it has another equally interesting aspect: we do not have to shave.

Furthermore, we can distribute the grafts differently, in this sense, on the frontal area it is interesting to have one graft for one hair while for other areas such as the tonsure it is much more interesting to have a high density per cm2, so a graft can have several to densify.

Tell me about the post-operative period of a hair transplant?

Over the years, Dr. SMARRITO and his team have improved the post-operative period.

Two main areas were worked on; on the one hand, post-operative sensations: patients often fear being in pain.

Hair transplantation itself does not hurt. A simple medical prescription, such as paracetamol, is often sufficient post-operatively. In some cases, when the sensations are greater, a morphine derivative in the form of drops will be enough to calm any pain. In fact, more than pain, it is a discomfort, which generally occupies the patient for a few weeks. Indeed, the area removed, whether in MCE or FUE SAPHIR, has been worked on and therefore skin tension is felt by the patient, in other cases, this tension can last several weeks. It does not prevent normal activity and we cannot strictly speak of pain.

In the first weeks, a helmet sensation, slight anesthesia of the scalp, is also felt. The second axis is a rapid return to active life. Depending on the grafted area, an edema may appear, it is generally zero for the tonsure area and can be more pronounced when the swellings are grafted. This edema can be completely absent, as more present with facial edema over a few days.

Here again, a specific treatment is put in place to limit it as much as possible. The after-effects are simple, precise instructions are given by Dr. SMARRITO post-operatively, such as the date when it is possible to shampoo and a strategy to remove the scabs as quickly as possible. For two months, the grafts will sleep, so there will be no new hair for at least two months.

After two months, the hair appears and is minimal, like down. At four months, the hair is more present, as with a short haircut, and the feeling of rejuvenation is already noticed by those around you. At six months, the hair is almost normal, and it is at 1 year that you can be creative in terms of haircuts.

What type of anesthesia do you offer for hair transplant?

The best position for performing a hair transplant is the semi-seated position, so the hair transplant will be performed under local anesthesia.

A simple injection is performed and we use modern means, such as laughing gas or premedications so that the sensations are not painful.

This anesthesia is light and therefore less risky than general anesthesia.

The exit takes place immediately after the end of the accompanied intervention, avoiding driving.

Can women have hair transplants?

Yes of course, it is possible to have a hair transplant in women, however this type of transplant in women has some particularities.

In fact, we cannot consider that there is a donor area in women who are not hormonally dependent, so the entire scalp behaves identically. If the hair falls out, there is no hair that resists, unlike in men where the crown resists and does not fall out. Thus, if a patient has a scarred area or an area where hair is missing, it will be necessary to move existing hair from the back to bring it to the area to be grafted.

It is therefore very important to do a preoperative assessment to be sure that it is not a disease that causes hair loss. I systematically see patients in consultation and prescribe a complete assessment in order to study the cause of the alopecia and characterize it.

 

Can multiple hair transplants be performed?

It is quite possible to perform several hair transplants, however, generally it is necessary to leave between 6 months and 2 years for the scalp and especially the donor area to recover. Thus for very large areas it will be possible to perform two hair transplants at a distance.

This allows large areas of alopecia to be covered. Note that if the patient has benefited from an FUE technique during the first hair transplant, we recommend repeating an FUE if the number of grafts has been limited to around 1000, and using a strip technique if more than 2000 grafts have already been taken.

Indeed, beyond 2000 grafts the scalp may appear sparse in the donor area.

Is it possible to cover the entire scalp in case of complete baldness?

If only the crown remains, it will be necessary to assess in consultation the possibility of doing grafts. Indeed, this will depend mainly on the height of the crown and also on the reserve and density of hair available. Sometimes it is possible to cover the entire baldness in a maxi session if the deficit is not too large, if on the other hand the deficit is too large and the crown lacks height we will often advise to carry out two grafts.

In some cases, it is also not recommended to do a hair transplant because the result will be too sparse and will not have the necessary rendering. Thus, in case of very advanced baldness, a consultation with a surgeon is very favorable.

Note that with FUE in cases of very advanced baldness, it can pose difficulties, in fact it is not necessary to reduce the donor area and FUE cannot, contrary to what can be read in advertisements on the internet, graft several thousand hairs in one session.

Is a transplanted hair identical to a natural hair?

Yes of course a grafted hair is identical to a natural hair since it is your own hair, note that often the hair is taken from the posterior area. This is an area where the hair is generally thicker.

So if we implant the gulfs, the implanted hair will be very slightly thicker than the hair of the gulfs. This is why we carry out a deconstructed implantation by mixing the quality of the hair to have a very natural and harmonious result. In no case except special request do we implant in line which gives an almost burlesque Playmobil type appearance.

Can white hair be implanted?

Yes, white hair is rather strong hair that we use in implantation, this allows to accentuate the capital.

In the presence of white hair, it is necessary to mix the graft to avoid an influx, a massive lock of white hair in a localized area.

We take this aspect into account and distribute the white hair thus collected so that the appearance is natural.

Is hair transplantation painful?

To answer this question, we need to discuss different hair transplants. First, we do everything we can to ensure that patients have a painless experience when it comes to hair transplants. The entire service is performed under local anesthesia and, being used to cosmetic surgery, we have significant resources to manage pain relief.

FUE is generally completely painless post-operatively, FUT or MCE techniques are generally not very painful, the pain being calmed by medical prescriptions. In cases of secondary grafts or catch-ups the operation can be painful because the scalp has already been worked on and the tensions can cause a painful sensation.

In the case of strips, a feeling of weight and tension may be felt between 3 weeks and 3 months depending on the sample and the patient's tolerance.

Is it possible to pay in installments without monthly fees?

Yes. It is possible to make a loan or a staggered payment without monthly fees. You can inquire at the secretariat. The financing organization is independent of SARL DR SMARRITO, guaranteeing transparency and a serene medical relationship. Payments can be made from 100chf per month, only administration fees are collected at the conclusion of the financing. The agreement is obtained after verification of the file by the financing organization.

How long should you stop exercising?

We recommend stopping sports activity for 10 to 15 days. In the case of intense sports activity or if you are a high-level athlete, it is possible to deviate from this instruction with the agreement of Dr. SMARRITO.

How long does a hair transplant last?

The duration of a hair transplant can last from 3 to 7 hours depending on the technique used (FUE is longer than FUT for the same number of grafts, MCE is faster).

We do not perform a 2-day collection, because this means that the number of grafts collected is excessive and risks leading to a lack of hair in the collection area.

What is the advantage of sapphire technique?

The sapphire technique is an improvement of the classic FUE. The mineral aspect of the tool helps reduce inflammation of the tissues around the grafts. The tool is very precise. The holes are smaller and better oriented. The appearance is more natural. This method would reduce the risks of infection (classically already very low).

How long does it take for hair to grow back after a transplant?

Whatever method Dr. Smarrito uses, the bulb is at rest for 2 months. That is to say, it takes 2 months for the new hair to come out in the form of down. Then at 4 months the hair begins to grow. It generally takes 1 year for the hair to be permanent.

Do you perform hair transplants without shaving?

Yes. I perform hair transplants without shaving. It is then easier to resume a professional or social activity after a few days. For my MCE transplant technique we practice "long hair". It is therefore preferable to keep some length before performing the transplant. In some cases of FUE it is necessary to shave the area or to perform a gradient. On the other hand, we never shave the implantation area or the entire skull as we can see on social networks, in Portugal or Turkey...

What do you think about medical tourism in hair transplant?

As a Swiss doctor, I benefit from medical tourism, as patients from all over the world have benefited from cosmetic surgery in my clinic.

So I have nothing against medical tourism.

Actually there are 2 streams, Patients who want quality choose a specialist in Switzerland at the right price. Patients who don't have the budget to do this in Switzerland, they go abroad or call on Swiss structures which bring them back abroad.

The level of surgery, security and hygiene is far above what is offered abroad.

Sometimes the prices are paradoxically close.

In any case, I never receive any complications from hair transplants coming from Switzerland. While I see every week patients disappointed or worried about the result of the transplant carried out in Turkey or Portugal by so-called specialized chains.

It is not the quality of the foreign surgeon that is problematic but the system that is purely mercantile. Some foreign clinics are ready to say, sell and do anything for money through a well-rehearsed marketing speech.
