Face lifting, rejuvenate without transforming

As FMH Cosmetic Surgeon, I do facelift. The facelift is still relevant today. It is a fantastic rejuvenation and beautification operation.

I am the founder of the RIVIERAclinic specialized in aesthetic medicine and injection expert. However, nothing replaces the facelift, neither injection, nor laser, nor tensor threads…

Indeed, the face lift is not an operation of the skin but of muscle anatomy that relaxes et fat volumes. When the lower face, initially thin and triangular, becomes square, with sagging, sagging and drooping jowls...The modern facelift is the only effective and stable solution over time.

THE NEW LIFTING: SCIENTIFIC REVOLUTIONS

NEW TECHNICAL TRENDS

Safety I Know-how I Efficiency with complete discretion

“I had the chance to be the operating assistant of Dr. MITZ, inventor of the modern SMAS facelift and head of Professor MIMOUN's department where 400 facelifts were carried out per year in PARIS.

Mon expertise in hyaluronic acid injection and my position as a surgeon at the LA PRAIRIE clinic (world leader for stem cells) allowed me to enrich the facelift technique with modern methods.

In 20 years, the facelift has been revolutionized: more effective, more stable, more discreet...more refined in the end. »

progress

Several operating stages follow one another to achieve a face lift, known as a juguo-cervial lift.

The skin is lifted and excess skin is removed without tension. Then the musculo-aponeurotic system is put back into tension (SMAS). The lower face is then lifted into the original anatomical position.

The droopy eyelids can be corrected at the same time. The cheeks are plumped without excess. The skin is worked by injection of stem cells or liquefied fat (nanofat). The operation is carried out under light general anesthesia. The duration is on average 2 to 3 hours.

Aftermath of surgery and hazards of lifting

Over the years, facelifts have become safer. Thus, technical evolution combines various gestures that are now lighter. The facelift is therefore less risky than 20 years ago.

Certain risks remain, such as hematoma, which is why the facelift requires one night of post-operative monitoring. Likewise, certain healing defects may appear in smoking patients.

The surgeon will explain these risks to you in consultation and will implement a technique to avoid them as much as possible.

Less resection thanks to fat micro-injections…

The new facelift

The very important fact in this modern technology is that we use autologous fat (from one's own body) which will be taken from the person, centrifuged and reinjected.

Now the technique exists in different forms: macrofat, microfat or nanofat. That is to say, we are going to have different densities of fats to meet different indications. THE lipofilling and micro-lipofilling allow you to add volume to the cheek area but also, for example, to dark circles.

Le nanofat (liquefied fat) allows the stromal vascular fraction to be supplied in quantity to a specific area to achieve cell renewal and improve the skin's cutaneous quality. This procedure is a specialist matter, it is sophisticated but it far surpasses all the other aesthetic medicine techniques that I perform on a daily basis.

Here is my opinion after 20 years of experience, the facelift has been revolutionized, it retains all these letters of nobility on condition of associating it with different gestures on the skin, the eyelids, the deep structure, the fat and to slightly plump up the volumes thanks to its own fat.

Facelift, you have to think about correcting and restoring the anatomy…

"I'm a fan of hyaluronic acid injections, but there is a time when the volume cannot compensate for the loss of anatomical structure and sagging skin. This is when you need to think about a facelift. » Dr. SMARRITO

Over the past ten years, the practice of classic facelift has been revolutionized. However, this facelift revolution went unnoticed, since at the same time there was the revolution in aesthetic medicine with injections.

The machines used radio frequency or others have a certain effect but it is only a matter of toning the skin.

The notched wires are also interesting. They allow the skin to be redistributed with an accordion effect without removing excess skin and really putting the structures back in place permanently.

La modern facelift revolution is due to the fact that the appearance must be very natural and we target the different parameters that cause aging.

Le looseness and  slip of the entire deep anatomical structure of the face is put back into place thanks to this facelift. The skin is then redistributed, redraped and we avoid major resections. Why avoid major resections? They gave this air stuck in the past, particularly quite visible in the American school of cosmetic surgery, some stars of which demonstrated the fact that excessive resection led to the opposite of the objective of facial rejuvenation.

Modern Face Lift

A faster, lighter face lift for a natural result

Dr Smarrito shares with you his more than 20 years of experience on the evolution of cervico-facial lifting techniques.

The facelift that I saw carried out 20 years ago is no longer at all the modern contemporary facelift. Indeed, the techniques have evolved but more than the techniques, the philosophies have changed.

The expert's opinion : Even if I realize more and morehyaluronic acid injectionsto delay the fact of doing a facelift, the facelift retains great interest in rejuvenation techniques.

Very clearly, in Switzerland, our local or international patients want a certain effect from the facelift but while keeping their expression and a natural side. In addition, the scars must be discreet and it must be easy to hide them.

Facelift under local anesthesia

The simplest and also the softest facelift is mini-facelift under local anesthesia. It will allow the excess skin to be tightened and the anatomical structures which have slipped to be slightly repositioned.

Indeed, facial aging is mainly characterized by a loss of fat volume, tissue sagging, a shift in the anatomical structure described by my Master, Vladimir Mitz and also distended skin.

In this context, it is entirely possible under local anesthesia by hiding the scars behind the ear to tighten the skin with a nice effect and by lightly plicating the anatomy to put it back in place.

Le facelift under local anesthesia lasts less long than the complete facelift but it has real effectiveness beyond the injections which only manage the volume and the tensor threads which ultimately have a fairly fleeting effect over time.

The classic facelift

The classic facelift, it's about jugo-cervical or temporo-jugo-cervical lift if the scar rises to the level of the temple to also manage the angle of gaze.

This lifting involves several stages, a time of skin detachment, a time of putting the anatomy back into place which is very important because it is the subsoil of the skin which slips and which gives this tired appearance and these undertows which are the jowl and the nasolabial fold. It is therefore quite important to anchor this structure so as to no longer have this sliding effect. Pulling only the skin has an effect but less present over time.

The classic facelift also allows the skin to be perfectly redrawn. Depending on the aspect of aging, whether it is wrinkled skin without excess fat, time will mainly focus on the recovery of this skin. The scars are then hidden inside the ear and behind the ear and perfectly concealed in the hair.

If, on the other hand, it is a face that has also become heavier with excess fat in the lower part, the facelift will be completed by a liposuction at the level of the chin which also allows the classic lifting effect which is the mandibular angle and the neck.

Very often, in these facelifts carried out around the age of 50-60, unlike the mini-facelift which can be carried out from the age of 45, a procedure is associated with blepharoplasty on the eyelids, also making it possible to correct the first signs of facial aging which are the excess skin on the upper eyelid and the appearance of bags on the lower eyelid.

FACE LIFTING FAQ

We answer the most frequently asked questions about facelift >>>

What is the difference between a mini facelift and a deep facelift?

There are several differences between facelifts. Thus, the deep facelift will allow a more complete correction of skin laxity and especially muscle relaxation. The deep facelift requires general anesthesia. The risks of complications (such as hematomas) are rare but present in this type of facelift. On the other hand, the result is often very demonstrative, while maintaining a natural side. It is classic to say that the deep facelift makes you look 10 years younger and stable over a period of 7 to 15 years.

The mini facelift is easier to implement. Classically, it takes place under local anesthesia. The separations are less important than in the cervico-facial lifting and the application of the muscles and the Smas and less deep. There are few risks when it comes to mini facelifts. it is in a way a simple technique, more efficient than the simple application of tensor thread or the use of Morpheus 8 (treatment of skin laxity with hifu).

Am I a good candidate for the mini facelift?

A consultation is necessary to answer this question. In fact, the indications for the mini and the real facelift will depend on several parameters. thus, one of the important parameters and the wish of the patient. Some patients want to have a very demonstrative result, so it is better to opt for a deep facelift. on the other hand, if it is a question of improving the oval of the face and treating moderate skin sagging, the mini facelift will be perfectly suited.

The surgeon carries out a consultation to make a diagnosis and assess the anatomy and needs. Thus, thanks to more than 20 years of experience, he will be able to advise you on a tailor-made treatment. Our philosophy is always to offer the simplest and least expensive technique provided that it is adapted to the objectives of surgical correction. Therefore, you should not choose the facelift method in advance since it is mainly conditioned by the appointment you will have with the doctor. finally, the budget can also be taken into consideration: a mini facelift is overall half the price of a full facelift. last point considered, the apprehension to carry out a mini facelift under local anesthesia and more moderate than a complete facelift so some patients will prefer not to undertake complete sedation and to have a very short convalescence with little impact on social life. be careful, as the results over time and the durability of the result are not the same, you should not ask a mini facelift to have the same duration as a complete facelift.

How long will the results of my facelift last?

The result of the facelift and its stability over time depends on several parameters. on average, mini facelifts are done every 5 to 7 years and full deep types every 10 to 15 years, of course lifestyle will strongly influence the result of the facelift in fact, for example, smoking reduces skin laxity and leads to a need to redo the facelift earlier.

one of the important parameters is skin elasticity. The surgeon goes to evaluate in consultation point some patients have skin sagging which becomes more pronounced over the years, even more so at the time of menopause. Thus, patients may require mini facelifts or a complete facelift closer together. Often the patient has skin laxity over the entire body, they are penalized on the inner side of the thighs, stomach and chest. The result, on the other hand, can be maintained thanks to HIFU type technologies such as Morpheus Pro and of course, by annual hyaluronic acid injections. Apart from this aspect of skin laxity, good weight stability also makes it possible to maintain the effect of the facelift, in fact the fatty relief on the face helps in some way to maintain tension. If you have lost a lot of weight, your face becomes hollow and your skin becomes looser. However, in most cases the facelift will give a stable result over several years.

What are the contraindications to facelift?

The contraindications of facelift are those of anesthesia, in the context of deep facelift. On the other hand, these are only contraindications to surgery in general for localized facelift under local anesthesia.

During the appointment with your surgeon, you will need to talk to him about any current medical treatment, especially illness. as well as treatments carried out on the face such as injections or laser treatments. It is also interesting to inform the surgeon of the presence of scars, particularly those due to accidents which could contraindicate the skin detachment of the facelift. From a general point of view, it is possible to have a facelift regardless of age as long as you do not have any severe pathology. anticoagulant or blood thinning treatments (AVK, aspirin, anti-inflammatory are often contraindications to the lifting technique. If you take this type of treatment, the doctor will contact your cardiologist or your family doctor to see if a temporary stop is possible. In most cases, it will be necessary to abandon the facelift and turn to less invasive techniques such as Morpheus. Cardiac, pulmonary pathologies, diabetes, HIV status and skin tumors are generally contraindications to facelift so as not to have an abnormal outcome or due to an unfavorable immunological background. Do not hesitate to come for a consultation, any surgeon is above all a doctor and will advise you on your medical history.

Will the scars be visible after my facelift?

This is the most frequently asked question to surgeons. Will my scars be visible after surgery? You should know that modern facelifts have evolved enormously in terms of scar rendering over the last 10 years. thus, the scars are more discreet, they are hidden in my hair inside and around the ear. In addition, the scars are fewer and shorter. many variations exist in facelift scars.

The surgeon at the first appointment will be able to show you the path of the scars in front of a mirror. there is no secret, scars are necessary they allow skin to be removed and access to the muscles to tighten it and thus restore normal anatomy in most cases the scars fade over a period of 6 month point they are very visible and only the first 15 days then much less the following months. It's not bad luck, the scars are not very favorable following surgery, various treatments exist such as microneedling or corticosteroid injection. finally if a scar is unfavorable it will always be possible at 6 months 1 year to do a touch-up in order to improve the scar often skin and millimetric resurrection and thus the mechanical force on the scar is weaker than during the first time point this allows to have finer scars during touch-ups. Despite everything, this necessity is rare because most scars become discreet very quickly.

Am I too old or too young to have a facelift?

Facelifts are generally performed between the ages of 45 and 65. Young patients may require a facelift, mainly due to sagging skin secondary to weight loss. For genetic reasons, some families have significant skin laxity, the oval of the face then sags from the age of forty.

In this context, if the treatment with Morpheus and the injection of hyaluronic acid are not sufficient, it will then be necessary to consider a facelift for young women. At the time of menopause, the face deepens, the muscles relax and skin laxity increases, thus creating the typical sagging with the appearance of the jowl. The face is less structured and heavier but it is then possible to do a facelift. In a way, facelift is profitable at any age. The deep facelift is particularly indicated during middle age and menopause.

We must not forget that gentlemen generally perform facelifts when they are older, when they are around 60 years old. Numerous publications show that patients are satisfied with facelifts and can have facelifts again (secondary facelift). These second facelifts are very effective and lighter to implement. In fact, there is no age limit for having a facelift; it is the state of health and fitness that determines the surgical indication.

Do I have to spend a night at the clinic?

As part of the classic jugo-cervical lift, your quote includes one night of hospitalization. This night of hospitalization is necessary for your comfort, your monitoring and to prevent possible abnormal consequences in the first 24 hours.

This mainly involves monitoring the appearance of a hematoma which requires simple evacuation, or even cauterization in the operating room.

As part of the facelift under local anesthesia (mini-facelift), the surgery is performed on an outpatient basis. After a short observation period (one to three hours, you can return home accompanied. For safety and liability reasons, you will not be able to drive after the intervention. Therefore, you must be accompanied or organize an accompanied outing. by taxi.

Is the cervico-facial lift painful?

No. This often surprises patients, but the cervico-facial lift is not painful. Patients tend to describe discomfort during the first 24 hours due to the semi-compressive dressing put in place. Given the slight swelling of the face, a feeling of discomfort is the rule. Furthermore, in rare cases a prescription for painkillers makes it very simple to avoid having this type of sensation. When prescribing, it is important to inform the doctor of the presence of a gastric ulcer, which contraindicates certain medications to combat edema.

Can you fly after a facelift?

It is possible to fly 24 hours after a localized facelift and 72 hours after a complete facelift. The surgeon mainly wants to check the dressing the next morning and over the first few days to detect the appearance of a hematoma. It is not a simple bruise, but rather an abnormal volume appearing on one of the cheeks and requiring drainage.

It is in this context of monitoring that the surgeon can request a delay in relation to the plane flight. It should also be remembered that a facelift involves threads which will need to be removed between 8 and 10 days depending on the work carried out.

When can I tie my hair back after a facelift?

It is possible to tie the hair the day after the facelift. This can also be practical for making dressings. On the other hand, creating a ponytail is not discreet for the first 10 days. Indeed, if you tie the hair the sutures will be more visible as well as the bruises (generally present the first week).

In the area behind the ear, the surgeon places the scars usually high up and in the hair. Thus, the ponytail is possible in daily life after a facelift.

Will the facelift correct my wrinkles?

The facelift mainly corrects sagging skin and sagging of the oval of the face. It is true that skin resection of a few centimeters of skin can improve wrinkles on the cheek. This is not strictly speaking the indication for a facelift. It is therefore not necessary to do a facelift to reduce cheek wrinkles.

If you want to reduce cheek wrinkles: monthly skin booster injections associated with picosecond laser treatment as well as regular treatments with Morpheus 8 will be more suitable. Please note that these treatments can be carried out after a facelift.

Likewise, the facelift does not correct wrinkles around the mouth. These wrinkles called bar codes can be corrected by combined treatments of mesotherapy and hyaluronic acid.

Is the “fox eye” lift recommended?

The fashion for “fox eyes” lifting is the subject of consultation requests, particularly for young and “trendy” clients. This eyebrow lift trend is old. Currently, it conveys an image of excessive transformation of the gaze, when practiced in the wrong hands. It should be noted that the eyebrow lift has existed for many years, it was actually invented by Doctor Fogli in Marseille who called this technique: the mannequin clamp.

It is entirely possible to do an external canthopexy or eyebrow suspension. We find this technique invasive and excessive. Furthermore, like any fashion, this suspended aspect of the look will go out of fashion! For our clients we recommend gentler techniques to accentuate the openness of the eyes. This may then involve combining superior blepharoplasty with the use of botulinum toxin and hyaluronic acid injections. For more information and to validate this strategy, a consultation appointment with the doctor is very important.

An aesthetic and technical approach will be offered to you.

The “One stitch Face Lifting” technique

This is a localized lifting technique. It is performed in 45 minutes under pure local anesthesia. In American terms, this is a “lunchtime” surgery technique.

This is actually a cheek pull. It is operated by desepidermization of a strip of skin at the level of the crow's feet. This method must be carried out by a surgeon since the plane must be perfect so as not to damage the frontal branch of the facial nerve. With little risk and almost no convalescence, this technique allows you to have the same effect as tensor threads, while being more stable over time. In addition, the cost is low, which promotes financial access to this surgery.

This innovative technique is perfectly suited to patients wanting a light effect without requiring a break from social life or convalescence as with a real facelift. It’s a competitor, tensor threads… which lasts longer.

How to reduce edema after a deep facial lift?

Most of the time, mini-facelifts do not cause facial edema. Thus, you will be affected by facial edema in the event of an SMAS type facelift. In order to reduce edema we advise you to:

  • wear a semi-compressive mask
  • raise face in bed
  • use ice packs without direct contact with the skin
  • the diet should be soft for the first 24 hours
  • use Helichrysum essential oil if necessary
