Gynecomastia, comfort regained…

Gynecomastia is a benign increase in the mammary gland male breasts due to an imbalance between the androgens and estrogen.

It is distinguished from theadipomastia ou lipomastia which is a simple localized fatty overload, requiring simple liposuction. Adipomastia is often linked to obesity with excess weight in the stomach area. In some cases, cancer must be ruled out, even if breast carcinoma is rare in men.

Gynecomastia is perfectly physiological at birth, adolescence and senescence. A assessment and consultation at Dr. Smarrito's clinic are necessary to rule out certain causes, such as thyroid, kidney or liver problems, medication or doping.

When the balance sheet is normal, it is a idiopathic gynecomastia. In recent cases less than 3-6 months medical treatment is possible, after the gynecomastia is fibrous and Only a well-controlled intervention will resolve the problem definitively.

Gynecomastia: Dr. Smarrito, an experience of the subject for many years

In most cases, it is not necessary to operate during adolescence unless the social and indirectly psychological context of the child is disrupted.

In this context, with the agreement of the various doctors involved in the file, the gynecomastia treatment is carried out. Moreover, it is generally extremely beneficial both in terms of the aesthetic aspect and the psychological comfort of the young child. With age, if this aspect has not regressed, it will not regress, we then speak of fixed gynecomastia.

It is sometimes possible to vary the fat but again given its very fibrous appearance, fat accumulated in the breast is extremely hard to remove. Finally, it is in this context that Dr. Smarrito after the initial diagnosis and the initial consultation will be able to perform liposuction or an cryoliposuction. In summary, breast liposuction allows fat to be removed while completely solving the problem of adipomastia (fatty breast).

My method of gynecomastia treatment

I offer you an effective method, which is not very painful and does not require time off work.

Mixed gynecomastia: gland and fat
Before After Gynecomastia
obesity and gynecomastia
breast hypertrophy

Surgical technique for gynecomastia:

This then involves making a micro-incision allowing liposuction of the area. Note that convalescence is marked by little pain but on the other hand bruising which is quite present for 15 days to 3 weeks.

Furthermore, if a glans is present, a barely visible scar in contact with the areola will be carried out in order to remove this gland which will itself be sent to histological analysis to be able to diagnose a breast cyst or tumor.

Le unilateral side on the other hand is quite suggestive of a doubtful lesion and it is then necessary to consult the doctor very quickly to carry out a hormonal radiological assessment and extract the tissue for rapid analysis.

Gynecomastia scars are generally discreet, in fact the liposuction cannulas are very thin. We place these scars so that they are as little visible as possible.

Should skin be removed when treating gynecomastia:

According to my experience of more than 20 years on the subject, it is quite rare to have to redrape the skin.

At the beginning of my experience, when I was Head in a pediatric aesthetic hospital, we tended to make scars in circles around the areola and the vascularization was then quite precarious with the risk of losing the nipple.

In the end, with my experience I realized that we could achieve very important liposuction, on average 0.4 liters sometimes up to 1 liter, with skin which rather than relaxing, tightens.

In some cases, it will be necessary to do a secondary skin lift at the cost of scarring but this is really no longer the rule at present.

Therefore, you can come for a consultation on this subject with Dr. Smarrito either directly for an opinion, or through your family doctor after hormonal and radiological assessment.

What is gynecomastia?

Gynecomastia is an increase in glandular tissue (mammary gland) in the breasts in men.

It can affect one or both breasts and can be caused by hormonal imbalances, medications, or other medical conditions. But, in most cases, no cause or disease is found.

What are the causes of gynecomastia?

Causes may include:

– Hormonal imbalances (increased estrogen or decreased testosterone).

– Taking medication (antidepressants, steroids, etc.).

– Diseases (cirrhosis, hyperthyroidism).

– Substance use (alcohol, marijuana, anabolic steroids).

Gynecomastia: medical facts to know
  • Gynecomastia is common in adolescents and older men. We can even consider it physiological at these ages.
  • Many drugs, so-called natural products, doping products, food supplements, and toxins must be researched.
  • Dr. Smarrito will prescribe an endocrine assessment mainly based on a blood test. The basic assessment combines studies of renal, hepatic, thyroid functions, as well as LH, B-HCG, estradiol and total testosterone.
  • In most cases, no cause is found, it is idiopathic gynecomastia. This is the most frequent clinical case.
  • When gynecomastia is less than 6 months old, drug treatment can cause the breast mass to regress; beyond that, it becomes fibrous and requires surgery.
What is the typical age for gynecomastia to occur?

At what age does gynecomastia usually occur?

Gynecomastia can occur at various ages:

– Newborn

– Puberty: common in adolescents due to hormonal changes.

– Adulthood: can develop at any age, often due to illness or treatment.

– Senescence: ovulation towards fibrotic tissue.

Is gynecomastia painful?

Gynecomastia may be associated with breast tenderness or pain, but in many cases it does not cause discomfort. Often it is the weight that causes this painful sensation.

In case of significant pain, you should consult a doctor to rule out breast cancer in men.

How to diagnose gynecomastia?

The diagnosis is generally established by:

– A physical examination during consultation.

– A medical questionnaire.

– Imaging tests (ultrasound or mammogram).

– Blood tests to assess hormone levels.

In practice, with my experience, I make the diagnosis by pinching the breast.

What sport stoppage does the doctor recommend after surgery?

On average, you have to stop sport for 15 days, except for athletes with a high level of training. I can also adapt the training depending on the patient and the surgery performed.

How to treat gynecomastia?

Treatment options include:

– Therapeutic abstention and monitoring (in the case of benign gynecomastia).

– Lifestyle changes (weight loss, reducing alcohol).

– Medications (anti-androgens or aromatase inhibitors).

– Surgery in cases that have not regressed in 6 months. Liposuction and/or opening for gland removal.

Can gynecomastia regress spontaneously?

Yes, in some adolescents, gynecomastia can go away without treatment within a few months.

What are the risks of breast enlargement surgery?

Risks of surgery include:

- Infection.

– Bleeding.

– Scars.

– Asymmetry of the breasts.

– Skin sagging in the absence of associated Bodytitis.

Is Gynecomastia a Sign of Cancer?

Although gynecomastia is generally benign, it can sometimes be associated with testicular tumors or other carcinomas including breast. A medical evaluation is essential. Beware of asymmetrical cases which are more suspicious.

Can a mammogram or ultrasound rule out cancer?

No, in my career I have discovered 2 cases of cancer at the time of histological analysis under the microscope, with normal mammograms. These were minimally invasive multifocal tumors.

Only microscopic examination (histological) is reliable to rule out the diagnosis. This examination is reimbursed by insurance funds.

Can gynecomastia be prevented?

Preventing gynecomastia may include:

– Maintain a healthy weight.

– Limit alcohol and drug consumption.

– Avoid medications known to cause gynecomastia.

What is the difference between gynecomastia and lipomastia - adipomastia?

Gynecomastia involves an increase in glandular tissue, while lipomastia refers to an accumulation of fat in the breast area without an increase in glandular tissue.

Can children develop enlarged breasts?

Yes, children can develop gynecomastia, often linked to hormonal changes.

Can you experience psychological distress because of growing breasts?

Gynecomastia can lead to emotional distress, low self-esteem, and issues with anxiety or depression. It is important to consult a healthcare professional for support if needed.

Many teenagers no longer dare to go swimming or wear tight clothing. Often obesity is a refuge to camouflage gynecomastia.

Throughout my career, I have literally saved teenagers who were psychologically sinking because of their breasts. This phenomenon is accentuated by social networks.

What are the risks of this operation?
Gynecomastia surgery, while generally safe, does carry some risks, including:
– Infection: As with any surgical procedure, there is a risk of infection at the surgical site.
– Bleeding and Hematoma: Bleeding may occur during or after the operation, sometimes requiring additional intervention. They are greatly reduced by combining liposuction.
– Scars: Incisions tend to leave scars, which may be more visible in some patients. The quality of scars varies among individuals.
– Breast asymmetry: There may be a difference in size or shape between the two breasts after surgery.
– Changes in sensation: Some people may experience temporary or permanent changes in breast sensation, including numbness or hypersensitivity.
– Healing problems: Some people may have healing problems, requiring additional care or interventions.
– Recurrence of gynecomastia: In some cases, gynecomastia can return even after surgery, in practice I have never seen it, even in doped patients.
– Anesthetic risks: As with any operation requiring anesthesia, there are risks associated with general or local anesthesia, including allergic reactions.
Is there a link between doping and gynecomastia?

Doping, particularly the use of anabolic steroids, is often associated with gynecomastia.

These substances can cause hormonal imbalances, increasing estrogen levels in the body, which can lead to excessive development of breast glandular tissue in men.

Athletes and bodybuilders using these products are more likely to develop this condition.

Can Soy Cause Breast Development?
Soy contains phytoestrogens, which are plant-derived substances with a chemical structure similar to estrogen.
In some studies, excessive soy consumption has been associated with hormonal changes, but evidence of its direct link to gynecomastia remains limited.
However, a balanced consumption of soy does not appear to pose a significant risk for the majority of individuals.
